Birth Certificates

The service window is open weekdays from 8:00 to 6:00 Monday - Thursday and 8:00 to 5:00 Fridays. except holidays.  Requests taken after 5:30 P.M. Monday - Thursday WILL BE PROCESSED THE FOLLOWING DAY, requests taken after 4:00 p.m. on Friday will be processed on the following Monday.   Some requests for records including those before 1955 may require 24 hours. Mail requests are usually completed within one week. Please wait at least three weeks after mailing your request before inquiring about its status. The Vital Records Office is located at:

The Weber-Morgan Health Department has certificates on file for births occurring in Ogden, Utah from 1909 to present, Weber County from 1958 to present, Morgan County from 1993 to present, and computer birth certificates for all of Utah from 1951 to present, and Utah County from 1905 to present. Birth certificates for newborn babies in Ogden are available about 20 days after the birth.

Certified copies of vital records may be obtained in person or by mail by that person or an immediate family member. For specific inquiries call 801-399-7130.

Additional copies ordered at the same time for the same record are $8.00 each.

All requests by mail require the following information: the type of record, the full name on the record, date and place the event occurred, names of parents, mother's maiden name, your relationship, and the reason for the request. Sign the request and include a return address, area code and phone number. A readable copy of a valid picture ID of the person applying for the record must also be included. Please make check or money order payable to Weber County Health.
Any request that is not completely filled out or is missing the valid copy of the applicants ID or the correct fee will be returned.
