What we offer

  • Birth Certificates: Ogden City from 1909 to present and all of Utah, including Weber and Morgan counties, from 1951 to present -- $20
  • Death Certificates: Ogden City from 1958 to present; Weber County since 1958, Morgan County since 1993, all Utah deaths from 2006 to present -- $18
  • All Utah divorce records from 1978–2010 -- $18
  • All Utah marriage records from 1978–2010 --$18
  • Additional copies ordered at the same time are $8

How to obtain a vital record

  • Same Day Service: Bring a completed application and valid ID to our service counter in downtown Ogden. Please remember to place your order at least one hour before our office closes each day or your order will be processed the following business day.
  • By Mail: You will need to send a completed application and include a clearly identifiable and easy-to-read photocopy of your valid ID. We ask that you do not send cash with your mailed items. Request forms can be downloaded using the links below. Please allow 2 weeks before inquiring about the status of your request.
  • Morgan County: We offer services in Morgan County by appointment only. Call (801) 399-7132.
  • Online: For your convenience, Weber-Morgan Health Department has partnered with VitalChek Network, Inc. a third-party vendor for online services. Additional service charges apply.
